Responsibilities:

1. Provide case management and appropriate level of care through risk assessment, care planning, and interventions to youths (low to medium risk) and their families.
2. Collaborate with schools and community stakeholders to implement a holistic intervention and supportive care plan for youths and families.
3. Conduct and facilitate psycho-education group sessions according to the curriculum provided to youths.

Requirements:

1. Candidate possesses minimum Degree/postgraduate in Social Work or Counselling.
2. At least 1-2 years' experience in working with youth and families will be preferred.
3. Knowledge and ability to case assess, engage, and carry out intervention plans in a timely manner.
4. Be able to work with clients in time-based programs to achieve desired outcomes.
5. Good team player with interpersonal skills in working with youth
