Job Description:

• Conduct patient intake interviews, financial assessments, and initial family evaluations for care arrangement cases, including home and agency visits.
• Provide support to patients and their families at the reception counter and through enquiry hotlines, offering information and coordinating with medical social workers to ensure seamless service delivery.
• Handle Medifund application processing and submissions at CMMAS, maintaining high standards of quality and compliance.
• Monitor and follow up on fund utilization and disbursement across various assistance schemes.\
• Oversee and ensure the quality and compliance of Medifund applications submitted by medical social workers from other departments.

Requirement:

• Diploma in Social work or related field
• Training provided
• Good communication and interpersonal skills
• Team worker
