Job responsibilities:

• Assessing and monitoring patients' health conditions at home.
• Administering medications, injections, and IV therapy as prescribed.
• Managing wound care, dressing changes, and post-surgical recovery.
• Independently assess patients, provide interventions, and escalate cases as needed.
• Communicate effectively with healthcare teams
• Provide caregiver training to ensure proper post-discharge care, maintaining high standards of patient safety and care quality.
• Maintaining accurate patient records and care plans.
• Ensuring compliance with healthcare protocols and infection control measures.
• Handling medical equipment and assisting with home-based treatments.

Job Requirements:

• Minimum a Diploma / Degree in Nursing
• Registered Nurse with the Singapore Nursing Board (SNB)
• Strong clinical skills with the ability to assess, plan, implement, and evaluate patient care.
• Good communication and interpersonal skills
• Ability to adapt to different situations.
