About the Role:

As a Medical Social Worker, you will work closely with patients, caregivers, healthcare professionals, and community partners to provide psychosocial assessments, counselling, care coordination, and financial assistance support. Key Responsibilities:

• Conduct psychosocial assessments to understand patients' emotional, social, and environmental needs.
• Provide counselling and supportive therapy to patients and families coping with illness, grief, trauma, or major life transitions.
• Develop and implement individualized care plans in collaboration with healthcare teams to ensure integrated support.
• Assist patients in accessing financial assistance schemes, community resources, and social services.
• Advocate for patients' needs, rights, and well-being in care planning and discharge arrangements.
• Participate in case discussions, multidisciplinary meetings, and family conferences.
• Provide crisis intervention, especially in cases involving abuse, suicide risk, or sudden illness.
• Maintain accurate and confidential case documentation in compliance with institutional and ethical standards.

Requirements:

• Degree or Postgraduate Diploma in Social Work from a recognized institution
• Eligible for registration with the Singapore Association of Social Workers (SASW)
