Summary:

• Working Hours: 5 days, office hour
• Working Location: West side - Day Care Centre
• Salary: $2,500 - $3,300 (based on experience)

Job Responsibilities:

• Facilitate Active Ageing Programmes (AAPs) and engage seniors via calls and visits to participate in the activities. Attendance should be registered in the system with every engagement conducted.
• Take charge of Community Screening (CS), ensuring seniors' needs are screened and particulars registered and updated. Seniors' needs should be rescreened upon discharge from the hospital or renewed every 2 years.
• Ensure Befriending and Buddying seniors are engaged in AAPs on a monthly basis.
• Assist seniors in any Information and Referral requests and log in the requests in the system.
• Collaborate with grassroots, partners and vendors on AAPs and events.
• Any other duties assigned by superior and reporting officer.

Requirements:

• Possess a diploma or above
• IT savvy and proficient in Microsoft office applications
• Knowledge of programme design, implementation, and evaluation
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team, and to manage multiple tasks and priorities
