Overview

We're looking for a compassionate and motivated individual to join our team as a Social Work Coordinator. In this role, you'll support patients and families with financial assistance and discharge planning, and assist Medical Social Workers with case management, translation, and home visits.

Key Responsibilities:

• Conduct intake interviews and financial assessments
• Assist with referrals and discharge planning
• Support casework and accompany patients on visits
• Handle administrative tasks and counter duties
• Collaborate on department projects and improvements

Requirements:

• Diploma (preferably in Social Work or related field)
• Strong people and problem-solving skills
• Comfortable with IT systems
