Responsibilities :
1. Casework and Counselling
• Conduct comprehensive psychosocial assessments for current and new admissions
• Develop, implement and review individualized care plans that remain responsive to evolving needs
• Provide case management and care coordination for residents and their families
• Deliver emotional support and therapeutic counselling to residents and caregivers
• Support families through transitions, including dementia diagnosis, care planning and end-of-life discussions
2. Supervision and Mentorship
• Provide clinical supervision, guidance and case consultation to Social Workers and Social Work Associates
• Model best practices in ethical decision-making, case formulation and client engagement
• Contribute to the professional development of the team through coaching, training and reflective practice
3. Interdisciplinary Collaboration
• Participate in multidisciplinary team meetings to ensure holistic, person-centred care for residents
• Partner closely with nursing, allied health, and medical colleagues in care planning and implementation
• Build and maintain collaborative partnerships with external agencies, including hospitals, government
bodies, community organizations and social agencies to mobilise resources for residents
4. Professional and Ethical Standards
• Uphold professional social work ethics and confidentiality in line with the Singapore Association of Social
Workers' Code of Ethics
• Ensure compliance with documentation, audit, and reporting requirements (e.g., MOH, AIC)
• Stay updated on current best practices and contribute to the refinement of care models within the Lodge
Requirements:
• Graduate Diploma or Degree in Social Work from a recognised institution
• Registered Social Worker (RSW) with SASW preferred
• Completion of Social Work Supervision training
• At least 8 years of social work experience in local restructured hospitals and nursing homes
• At least 5 years of experience in supervisory capacity
• Familiarity with the National Social Work Competency Framework and person-centred dementia care
• Strong clinical social work skills in casework, counselling and crisis management
• Effective communicator and compassionate listener, especially in sensitive conversations
• Comfortable navigating complex family dynamics and care transitions
