Job Responsibilities:

• Deliver comprehensive case management services-including assessment, care planning, intervention, and evaluation-to support patients within both hospital and community settings.
• Design, facilitate, and lead group sessions aimed at fostering social support networks for specific patient populations.
• Partner with community stakeholders to identify local needs and implement initiatives that enhance patients' psychosocial health.
• Contribute to the development and execution of programs aimed at improving care quality and advancing social work practices.
• Provide mentorship, supervision, and training to junior social workers, support staff, and social work students.
• Organise and conduct psychoeducational talks, workshops, and seminars for patients, caregivers, and the general public.
• Assist in research activities, including data gathering, analysis, and the preparation of presentations and publications.
• Support the administration of departmental operations, including service delivery, resource management, and project coordination to meet quality benchmarks and departmental objectives.

Job Requirements:

• Degree in Social Work from an accredited university
• Preferred with at least 1 year working experience in Healthcare
• Strong team player with excellent interpersonal skills.
• Empathetic and culturally sensitive, with a strong commitment to patient-centred care and advocacy.
• Adaptable and resilient, with the ability to handle emotional situations and support patients through psychosocial challenges.
