About ICCP

Today, senior care services are structured around different health archetypes-AACs support active seniors, SCCs, DRCs, and HT focus on rehabilitation and frail seniors, while HPC aids those who are home-bound. While these services provide crucial support, seniors often struggle to navigate the care system, finding the right providers, and transitioning seamlessly between care options as their needs evolve.

The ICCP model was created to redefine coordination in senior care, ensuring services work together seamlessly and that seniors experience continuity and ease in their care journey. As ICCP Team Lead, you will be the driving force behind this effort, establishing systems that empower seniors to access the right care, at the right time, without unnecessary delays or confusion.

Your Role as ICCP Team Lead

As the ICCP Team Lead, you will spearhead the groundwork and operational setup for ICCP in our assigned subregions. Leading a team of care assessors-whether from Presbyterian Community Services (PCS) or assembled across service providers within the sub-region-you will oversee the smooth execution of ICCP, ensuring:

✓ A unified experience for seniors - minimizing fragmentation and ensuring smooth service transitions

✓ Standardized assessments - so every senior's needs are evaluated consistently

✓ Shared care plans - fostering seamless collaboration among service providers

✓ Timely access to services - eliminating gaps in care that could hinder a senior's well-being

✓ Continuity of care - so seniors remain supported no matter how their needs evolve

With a hybrid work arrangement, your base will be in PCS centers and available spaces within our subregion partner sites-enabling flexible collaboration with stakeholders.

Your Key Responsibilities

➢ Design & Build ICCP Operational Systems

• Establish policies, processes, and frameworks for a robust, integrated senior care system, ensuring all stakeholders are aligned

• Work closely with service providers to shape a collaborative care model

➢ Strengthen Partnerships & Collaboration

• Lead partnership engagements with ICCS and non-ICCS providers, fostering a strong, unified network taking a human centric approach

• Serve as the bridge between long-term care (LTC) services and community partners, ensuring seniors receive holistic care

➢ Oversee Service Delivery & Governance

• Provide secretariat support to the ICCP Steering Committee-uniting service providers within the subregion

• Train staff in the use of InterRAI assessment systems, equipping them with essential and standardized tools for care planning

Who We're Looking For

✓ Senior Care Expertise - Good understanding of Singapore's senior care landscape, including service providers and care models

✓ Stakeholder Engagement Pro - Experience in establishing partnerships and navigating multi-agency collaborations

✓ Strategic Leader - Ability to design systems, formulate processes, and manage standard operating procedures

✓ Social Work & Case Management Knowledge - Prior experience in supervision, case management, or program coordination will be a plus though not compulsory for application

✓ Collaborative & People-Centric - Strong interpersonal and communication skills with a track record of leading cross-organizational projects
