Job Scope

• Assess, develop, implement, and evaluate client care plans to meet individual care needs
• Lead and mentor a team of Community Caregivers to ensure high-quality service
• Provide guidance and resources to help clients and families navigate care challenges
• Facilitate access to supplementary care resources as required
• Conduct home visits to assess living conditions and deliver in-person care
• Manage administrative tasks, including documentation and reporting, to maintain care standards
• Support community engagement initiatives, including weekend/holiday events when required

Job Requirements:

• Registered with Singapore Nursing Board
• Diploma / Degree in Nursing
• At least 1 year of nursing experience
• 5-days work week
