Key Responsibilities

• Conduct clinical assessments to determine patients' functional abilities and therapy needs
• Develop and implement individualised treatment plans to improve clients' ability to perform activities of daily living (ADLs)
• Deliver therapy sessions across various settings (inpatient, outpatient, community, home visits)
• Educate patients, caregivers, and family members on therapy techniques and home-based activities
• Work collaboratively with multidisciplinary teams (e.g., physiotherapists, doctors, medical social workers)
• Monitor patient progress, maintain accurate documentation, and adjust treatment plans as needed
• Support transitional care and discharge planning in line with continuity of care practices
• Participate in community outreach, rehabilitation programmes, or assistive technology recommendations
• Adhere to ethical standards and the Allied Health Professions Council (AHPC) guidelines

Requirements

• Bachelor's or Master's degree in Occupational Therapy (recognised by the AHPC)
• Full registration or eligibility for registration with the Allied Health Professions Council (AHPC), Singapore
• Relevant clinical experience in geriatrics, paediatrics, mental health, neurorehabilitation, or community care preferred
• Strong interpersonal and communication skills
• Able to work both independently and within a multidisciplinary team
• Passionate about enabling patients to achieve meaningful outcomes
• Experience in community rehabilitation or transitional care programmes
• Familiarity with assistive devices, home modifications, and caregiver training
• Knowledge of local healthcare initiatives (e.g., Healthier SG, Ageing-in-Place)
