Job Description
The Senior Enrolled Nurse plays a key role in overseeing and coordinating home-based care services, including Home Personal Care, Home Medical, and Home Nursing services. This position involves managing service scheduling, ensuring high standards of patient care, handling patient feedback, and supporting the team in delivering quality elderly care.
Role & Responsibilities
1. Patient Care & Clinical Responsibilities
- Provide basic home nursing care to elderly patients in their home environment.
- Conduct home visits to perform biopsychosocial and environmental assessments, including MBI (Modified Barthel Index) assessments.
- Develop, implement, and regularly review individualized nursing care plans, ensuring appropriate nursing interventions and rehabilitation procedures.
- Deliver specialized nursing care and preventive health procedures as needed.
- Evaluate patients' responses to care interventions and recommend necessary modifications to care plans.
- Provide patient and caregiver education to support self-care and improve health outcomes.
2. Coordination & Team Collaboration
- Plan and coordinate Home Care and Home Personal Care schedules, ensuring efficient service delivery.
- Communicate and collaborate with interdisciplinary team members, updating them on changes in patients' status and care needs.
- Serve as a primary point of contact for patients and caregivers, addressing concerns, providing guidance, and ensuring a positive care experience.
3. Customer Service & Complaint Management
- Handle patient feedback and complaints professionally, investigating concerns and implementing corrective actions as necessary.
- Ensure effective communication with patients, caregivers, and team members to foster trust and confidence in the care provided.
4. Administrative & Documentation Duties
- Maintain accurate and timely clinical documentation, including patient progress notes, assessments, and reports.
- Ensure compliance with nursing standards, regulatory guidelines, and best practicesin home-based elderly care.
5. Perform additional duties as assigned by the Nurse Manager.
Requirements
Qualifications & Experience
- NITEC Certificate in Nursing from a recognized institution as approved by the Singapore Nursing Board (SNB).
- Valid SNB nursing license and adherence to professional regulations.
- At least 3 years of nursing experience, preferably in the public healthcare or nursing home sector.
Knowledge & Skills
- Strong clinical skills and experience in home-based care services preferred.
- Demonstrates good communication and people engagement skills, preferably with the ability to speak local languages and dialects.
- Good communication & interpersonal skills to interact effectively with patients, caregivers, and healthcare teams.
- A motivated, resourceful, and mature team player who enjoys working with the elderly.
- Positive attitude, people-oriented, and has a pleasant personality.
- Proficient in IT, including the use of email, calendar, shared drives, common software, and smartphone applications.
- Strong leadership and coordination skills to manage scheduling and interdisciplinary collaboration.
Work Schedule
- 5-day work week (Monday to Friday)
- Working hours: 8:30 AM - 5:30 PM
- Off days: Saturdays, Sundays, and Public Holidays ( However, attendance at organization events on weekends may be required when necessary. )
