As a Care Manager, you will be responsible for the following duties:

• Identify and assisting patients who require care management plan
• Monitor residents' daily schedule and care plan.
• Monitor and tracking patient's progression to ensure optimal clinical / non-clinical outcome
• Engage with residents and family members to determine satisfaction with quality of care
• Work with clinical team and stakeholders to provide quality care to the resident
• Documenting and ensure confidentiality of patient's records
• Participate in quality improvement projects
• Perform / participate in outreach programs or activities
• Any other duties required within the team

Requirements:

• At least Diploma / Degree in Nursing with valid SNB registration
• Candidate with Advance Diploma in specialty area will be a plus
