Responsibilities:

• Case management
• Provide counselling to clients
• Conduct assessment on clients' needs and conditions including but not limited to psychosocial and financial assessments
• Make referral to relevant agency; Attend to referral cases
• Conduct training for caregivers and internal staff
• Participate in in-house training

Requirements:

• Medical Degree in Social Work
• Candidate with at least 1 year of experience is preferred
• Excellent interpersonal and communication skills.
• Strong ability to work in a multidisciplinary team and collaborate effectively
• Compassionate, empathetic, and able to handle sensitive cases with professionalism
