Responsibilities and Duties:

• The Care Coordinator is to conduct home visits to provide psycho education, support and linkages cum care-coordination of services to clients and caregivers.

• Conduct assessment of patients for medical, psychological, social, environmental and vocational needs.

• He/She has to be a team player to collaborate with a group of Allied Health Professionals, doctor and nurses.

• Provide psychological input and expertise during multi-disciplinary meetings.

• Provide trainings to team / caregivers as necessary.

• To conduct Mental Health support group for patients and /or caregivers.

• To carry out projects and duties assigned by supervisor or Head of Department.

Job Requirements:

• Registered Nurse with Singapore Nursing Board.

• Possess a RMN/Adv Dip in Mental Health or other relevant qualifications (e.g. social work, counseling, psychology).

• Experienced in psychiatric setting for at least 3 to 5 years (Community psychiatric clinic / IMH) will be advantage.

• Has experience using psychotherapy with mental health patients.

• Passion to work in the community.

• Possess strong organizational, interpersonal, communication and writing skills.

• Good computer literacy skills - MS Office etc.

• Familiar with healthcare environment.

• Ability to multi-task, work independently and collaborate as a team.

• Will be advantage to be able to converse with English, Chinese, Malay & Dialect-specking clients.

Passion in managing people with mental health disorders.
