Job Description :

• To work with medical social worker on referral cases
• Organize home visits and fieldwork under the guidance of MSW
• Facilitate communication and provide administrative procedure to client and their families.
• Help client apply for financial assistance application and AIC Long Term Schemes
• Collaborate with doctors, nurses and other healtcare professional to develop and implement comprehansive care plans
• Maintain an updated register and records of client
• Respond to inquiries regarding referrals, services, and charges.
• Ensure timely and accurate confidential documentation of client assessments, care plans, and referral statistics.
• Connect client and families with community resources and assist to provide with information
• Assist with data entry and filing

Job Requirement :

• Min Diploma/Degreen in Social Work
• At least 1 years relevant healthcare industry experience
• Good interpersonal and problem-solving skills
• Proficient in English and able to converse in dialects or other languages
• 5 days work week
• A caring, positive and self-disciplined individual, ability to work independently as well as within a team.
• Enjoys in a Geriatric Care
