Job Scope

• Evaluate, design, deliver and monitor client care needs
• Oversee and guide a team of Community Caregivers
• Create, execute and revise individualise client care plans
• Provide support and educate client
• Arrange client for additional care or services as needed
• Perform home visitation for client
• Assist with administration work and reports
• Oversees resources for nursing operation
• Support the execution of occasional events, which may take place on weekends and public holidays.

Job Requirements:

• Registered with Singapore Nursing Board
• Diploma/ Degree in Nursing
• At least 1 years of nursing experience
• 5-days work week
