KAMPONG KAPOR COMMUNITY SERVICES
- Safe & Strong Families-Preservation (SSF-P) as per programme requirement. (Duties included but are not limited to the following:)
Casework - Engagement & Assessment
a. Engaging CYP and significant others (e.g. family, caregivers, etc.) in a therapeutic manner to understand their needs.
b. Managing complexity of cases with trauma and adverse life experiences.
c. Developing professional and engaged relationship with CYP, family and other key service stakeholders (e.g. Medical Professionals, counsellors, Family Service Centres (FSCs), etc.).
d. Conducting needs assessment and risk assessment in accordance to the service framework (e.g. Use of tools such as SDM, Partnering for Safety, etc.).
e. Providing home-based intervention and support to families.
f. Demonstrating appropriate use of interpersonal skills to challenge families in an appropriate manner.
Casework - Case Review & Documentations
a. Implementing and following up on care/ case plans and interventions to determine the suitability of care/ case plans in addressing client's needs and modify them where necessary.
b. Preparing reports using the prescribed tools (refer point 1d above) on a bi-monthly basis.
c. Completing all necessary documentation for all clients, including case reviews, discharge summaries and other ongoing records.
Casework - Goal Setting & Intervention Plans
a. Developing appropriate preservation case plans that is inclusive of the CYP and family's opinions.
b. Conceptualizing and identifying appropriate approaches and other intervention strategies that achieve the overall care plan of each client and developing appropriate care/ case plan for CYP.
c. Working closely with multi-disciplinary team and/ or agencies to deliver care/ case plans.
d. Effectively use approaches to ensure that the emotional, social, behavioural, developmental, therapeutic and educational needs of clients are met through the development, implementation and timely review of individual safety and care plans.
e. Identifying appropriate interpersonal skills to utilise when managing and responding to conflict to de- escalate tension.
f. Applying appropriate interpersonal skills to develop professional and engaged relationship with other key service stakeholders for cases within the community.
g. Negotiating with systems to ensure safe and timely step-down to community services.
2. Professional Development and Education
a. Reviewing and reflecting on own practices.
b. Engaging in supervision sessions with the SSF-P supervisor/ MSF appointed agencies.
c. Proactively soliciting feedback from others and using it to enhance individual performance and practices.
d. Understanding how diversity in the practice/ client situation (e.g. culture, religion) impacts the service and the ways practitioner should engage with the clients.
e. Attending professional development courses to acquire professional knowledge and skills.
3. Others: eg; Research
a. Playing an active part in the evaluation of the SSF-P programme and other therapeutic models and programmes.
b. Undertake any other duties given by the Supervisor and Executive Director.
