The role of an Advance Care Planning (ACP) Programme Coordinator involves facilitating appropriately staged ACP discussions with patient and/or their family members, to document decisions reflective of patient's wishes and plans for future care. You will work closely with multidisciplinary professionals and community partners to facilitate holistic care for patients. You will also contribute to the development and implementation of ACP across SGH campus, which includes areas such as project management, quality improvement, research and outreach to the public and other healthcare professionals.

Job Requirements
  • Minimum a Degree in relevant field with at least 2 years of working experience in a healthcare setting (Preferably background in social work, counselling, psychology or nursing)
  • Possess strong organizational, coordination, interpersonal and communication skills
  • Prior training in National Advance Care Planning Facilitation will be an advantage
  • Possess proficient computer knowledge (Microsoft Office - Word, Excel, PowerPoint)
