Job Title: Social Worker/ Caseworker

Job Description:
- Assess the community health and social care needs
- Coordinate support and service placement (where required)
- Develop and pilot work flows, protocols and process that facilitate the integration of social and health care for residents
- Develop and implement calendar plan of social and health activities for seniors, with seniors to promote a community climate of multi-racial tolerance, harmony and co-operation among residents
- Employ strengths-based practice model to work with the service user
- Identify, stocktake and assess the strengths and limitations of community assets, including other Community Care service providers
- Make available opportunities to encourage service user's participation in programmes and activities
- Network with relevant community organisations to develop and strengthen programmes and services for the service users
- Participate in case management discussions with external service providers
- Perform community profiling and identify the needs, interests and capabilities of residents through methods including but not limited to door-knocking, surveys and interviews
- Provide opportunities for service users to remain connected to the community
- Assist in the supervision and/or training of persons/students on placement
- Conduct presentations as and when required to external parties
- Keep up-to-date statistics and records of the services rendered
- Provide guidance and on-the-job training, including SOP sharing, to staff or volunteers
- Perform general admin duties
- Other duties as assigned by the Programme Manage
Job Requirements:
- Degree in Social Work, Psychology or Counselling, or a Graduate Diploma in Social Work, Psychology or Counselling.
- Willing to comply with the Standard Operating Procedure when discharging duties.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team, and to manage multiple tasks and priorities.
- Passionate working with elderly clients/ residents in community/ welfare homes setting.
Compensation:
- Working location: Buangkok/ Eunos
- Working days: Mon to Fri, Office hours.
- Benefits: annual leaves, medical leaves, medical and insurance coverage.
