Reporting to the Chief Executive Officer, you will:

• To provide and lead social work intervention and management of existing /new programs of Cheshire Home and take on the supervision of junior social work staff.
• To manage ongoing cases of PWD to promote their quality of life and support community living to the best of their abilities.
• To provide regular updates of next of kins (NOKs) on the beneficiaries and families and to coordinate admissions and discharges.
• To maintain documentation of case history in CMS (care system) and regular submission of reports to MSG, SGEnable annually, including EPES, Case Central Management when implemented.
• To support and present cases to the Admission & Review Committee for evaluation and approval for fee charges and obtain approval for Home subsidies for low-income clients.
• To prepare and update Individual Care Plan (ICP) for all residents and Day care clients, including new clients from Training in Independent Living (TIL) if successful in obtaining funding.
• Liaise with MSWs of referring organizations and/or Doctors for relevant medical information especially on present treatment if these have not been given in the application form.
• Conduct regular psycho-social groupwork with residents when required and participate in multi-disciplinary team discussions with Nursing, Therapy and other allied healthcare for holistic interventions.
• Explore job or placement opportunities for the beneficiaries for those who have interest in finding external employment.

• To contribute to program research work, especially for new programs at Singapore Cheshire Home.

Requirement:

• Registered Social Worker with Bachelor's or Graduate Diploma in Social Work.
• More than 8 years of experience in Social Service Agencies, MOH or NH in similar capacity.
• Have undergone training in Suicide intervention, management of violence and abuse.
