TASEK ACADEMY AND SOCIAL SERVICES LIMITED
1. Duties
i. Case Management
a) Conduct regular assessment of youth and caregivers using appropriate assessment tools
b) Develop youth-centric case plan for each youth identified according to the programme protocol for follow ups
c) Monitor and track attendance at schools and address any barriers to attendance (which may require engaging parents via home visits or other youth or parent engagement platforms)
d) Attend and convene case conferences, where necessary, with school teachers and mentors, MDT, and other agencies (e.g. Social Service Offices, Family Service Centres, etc.) to coordinate support for youth and their families at a systemic and case level (e.g. ComLink+)
e) Provide last mile delivery of support to families (e.g. accompanying parents and their youth for further health checks)
f) Support and ease transitions across different school stages, and withdrawals from the programme
g) Document case plans and notes of meetings/case conferences
h) Support the needs of other children between in the family if they are not known to any social services
ii. Outreach
a) Organise, execute and participate in outreach activities to find and recruit potential disadvantaged families or families who need assistance together with volunteers.
b) Record the outcomes of outreach to determine status of targeted youth.
iii. Intake and Referral
a) Receive and process referrals from a variety of sources.
b) Seek consent from and determine the eligibility of families for programme components.
c) For families in the region who are not eligible for the programme, to refer them to relevant social service agencies.
d) Collate and maintain accurate and complete set of data on youth enrolled to the programme.
iv. Engagement
a) Support the organisation mapping of community needs and assets.
b) Collate needs from families overseen by Tasek for possible sponsorship or donation opportunities.
c) Assist with disbursement and tracking of sponsored items to youth and families.
2. Job Requirements
a) Bachelor's degree in Social Work, Social Sciences or related field.
b) Keen to work with disadvantaged youth and families and willing to go the extra mile to support families in need
c) Good writing skills for case management andcomfortable with data collation for reporting purposes.
d) Strong organisational skills, with an ability to manage multiple projects and meet deadlines effectively.
e) Excellent communication and interpersonal skills, with an ability to work collaboratively with various stakeholders including parents, caregivers, teachers, and community partners.
f) A passion for uplifting individuals and families in vulnerable or disadvantaged situations, which involves a keen interest in understanding and forming relationships with individuals in need ofsupport while adhering to professional boundaries.
g) Proficiency in Microsoft Office, familiarity with assessment tools such as FAST, CANS and platforms such as Case Connect is an advantage.
h) Able to work on irregular hours due to home visit scheduling.
