Care Coordinator

Working Hours:
Monday to Friday - 8:30 am - 5:30 pm
️ Saturday - As required for events or support
️ Monthly after-work team meeting
Job Purpose:
To support the smooth delivery of daily programs and personalized care for seniors at GlowCare Club, ensuring clients' physical, cognitive, emotional, and social well-being through the Glow Method. The role focus on care coordination and client engagement responsibilities.
Key Responsibilities:
1. Program Support
• Assist in setting up, conducting, and cleaning up after activity sessions (cognitive, physical, social, and creative).
• Motivate and encourage clients to participate actively in daily programs.
• Observe clients' responses and progress during sessions and report to the CM or therapist.
2. Care Coordination & Client Support
• Welcome and assist clients upon arrival; ensure comfort and readiness for daily activities.
• Monitor clients' well-being, hygiene, safety, and mobility throughout the day.
• Support grooming, toileting, and feeding assistance when required.
• Coordinate with caregivers regarding attendance, health updates, and individual needs.
• Maintain accurate daily attendance, meal, and observation records.
3. Administrative & Communication Duties
• Record clients' progress notes and share observations with the team.
• Assist in scheduling or updating client programs and therapy sessions.
• Help prepare reports and materials.
• Participate in team meetings, training, and monthly after-work sessions for continuous improvement.
4. Environment & Safety Maintenance
• Maintain cleanliness and organization of therapy and activity areas.
• Ensure equipment and materials are well-kept and safe for client use.
• Support emergency or safety drills and follow infection control protocols.
5. Event & Program Support
• Assist during weekend events, outings, or caregiver workshops when required.
• Help in festive or thematic activities to enhance clients' enjoyment and social interaction.
Personal Attributes:
• Compassionate, patient, and positive attitude toward seniors and persons with dementia.
• Good teamwork and communication skills.
• Observant, responsible, and detail-oriented.
• Physically fit to support clients in mobility and exercise sessions.
