Responsibilities

• Assist the care team in coordinating patient care and enhancing overall care delivery.
• Collaborate with nursing and allied health professionals to ensure patients and their families receive appropriate care options and support.
• Assess patients' needs to contribute to the development and improvement of personalised care plans.
• Take part in community outreach and engagement initiatives.

Requirements

• A degree in a health-related discipline.
• Experience in the healthcare or social service sector will be advantageous.
• Background in care or case management is preferred.
• Self-motivated, positive, and able to work well in a team.
• Strong commitment to service quality and continuous improvement.
