Duties & Responsibilities:
- Conduct biopsychosocial assessments and formulate Individual Service Plans (ISPs).
- Provide counselling to residents and families, including crisis intervention when required.
- Prepare timely case notes, progress reports, and submissions in line with organisational and MSF requirements.
- Provide Information & Referral (I&R) services to link residents and families with appropriate community resources.
- Participate in the planning, implementation, and evaluation of programmes and activities that support rehabilitation and reintegration.
- Assist in organising social outings, family sessions, and community-based programmes.
- Support outreach initiatives to raise awareness of services available.
- Work collaboratively with counsellors, psychologists, and other professionals to ensure holistic care for residents.
- Advocate for residents' needs and rights during case conferences and multidisciplinary discussions.
- Engage families to strengthen support systems.
- Participate in training, supervision, and reflective practice sessions.
- Share knowledge, best practices, and learning with team members to improve service quality.
- Uphold professional ethics, confidentiality, and service standards in all interactions.
- Undertake additional tasks as assigned by the Senior Social Worker.
