Job Responsibilities

Coordination and Collaboration:

• Facilitate collaboration among Senior Care Centre, Active Ageing Centre, Home Personal Care+, and Home Therapy providers to establish roles, responsibilities, processes and workflows, ensuring smooth implementation and delivery of ICCP services and achieving ICCP KPIs for the specified sub-region.
• Lead ICCP partners' leadership meetings to set strategic directions, review performance and address service gaps, and evaluate resource opportunities.
• Organize ICCP partners' operational meetings to plan and manage the effective delivery of the four core eldercare services in the sub-region.

Reporting & Administrative:

• Oversee timely submission of progress reports and claims to MOH and AIC.
• Act as the main point of contact for ICCP-related administrative matters.

Job Requirements

• Minimum Bachelor's Degree in healthcare management, social work, gerontology, or related field.
• Minimum 4 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships.
• Exceptional leadership, communication, and interpersonal skills, coupled with the ability to plan, execute, and collaborate effectively with diverse stakeholders
• Able to commit to a 2 year contract
