Job Scope:

• Facilitate structured and empathetic Advance Care Planning conversations with patients and their families.
• Educate individuals on goals-of-care, advance directives, and other end-of-life care options.
• Work collaboratively with an interdisciplinary healthcare team to ensure ACP documents are accurately integrated into patient records and effectively communicated.
• Actively contribute to the development and rollout of ACP programs and initiatives within the organization.
• Uphold the highest standards of confidentiality, ethics, and organizational policy in all interactions.

Job Requirements:

• Min degree in Social Work/Counseling or relevant
• Holds an ACP Certificate
• Working hours: Mon to Fri, 9am to 6pm (open to work on Weekends)
