Social Work Coordinator
(Healthcare / Community Care Sector)

Responsibilities:
-Provide financial assistance to patients and their families.
-Conduct intake interviews and financial assessments.
-Offer assistance and information to patients and family members through reception and inquiry lines.
-Liaise with internal and external stakeholders to ensure smooth and effective service delivery.
-Process financial assistance applications and ensure compliance with policies and procedures.
-Prepare reports, monitor departmental funds, and provide administrative support.
-Perform other duties as assigned by the supervisor.

Requirements:
-Higher Nitec or Diploma in Community Care & Social Service (or equivalent).
-A good team player with strong interpersonal skills.
-Able to work independently with minimal supervision.
-Passionate about serving the community and making a difference.
