Job Description :

• Provide comprehensive case management and supportive counseling for seniors and their family caregivers.
• Conduct psycho-social and financial assessments to inform personalized care plans and clinical interventions.
• Plan, develop, and implement community programs and support groups tailored to the needs of seniors and caregivers.
• Facilitate therapeutic support groups and conduct home and hospital visits as part of ongoing client support.
• Serve as a key liaison, building and maintaining strong networks with relevant agencies and stakeholders to advocate for client welfare.

Job Requirement :

• Min Diploma in Social Work
• At least 3 years in Social Service Sector
• Good interpersonal and problem-solving skills
• Proficient in English, bilingual candidates with fluency in local dialects as role involve to communicate with dialect speaking clients (ability to translate dialect to English)
• Working Hours: Weekdays 9am to 6pm
