Job Description

• Assess the needs of seniors requiring care or services and develop care plans.
• Assist in the creation and implementation of programs for seniors, including data collection and basic analysis.
• Coordinate care for seniors with relevant stakeholders.
• Document case notes.
• Facilitate appropriate service referrals for seniors.
• Participate in case discussions with external service providers.
• Provide case management services.
• Conduct needs assessments for seniors in the community.
• Prepare data for submission to relevant authorities.

Job Requirements

• Degree in Social Work, Psychology, or Counselling
• Preferably 1 of experience in the social service sector or a related field
