About the Role

You will play a key role in managing referrals and supporting case oversight for individuals in shelters and institutional care. The role involves close coordination with internal teams, external partners, and community stakeholders to ensure timely intervention, efficient placement, and smooth reintegration for clients.

Key Responsibilities

Referral Management
• Oversee the end-to-end referral process to ensure timely and effective case handling.
• Coordinate with internal departments and external agencies to facilitate smooth referral pathways.
• Ensure proper documentation, tracking, and follow-up of all referrals.
• Review and streamline referral processes to maintain relevance and efficiency.

Monitoring Capacity and Vacancies
• Engage partners and stakeholders to support rough sleepers and vulnerable individuals.
• Monitor capacity and vacancies across S3Ps, Transitional Shelters (TSes), and Welfare Homes (WHs).
• Collaborate with the Data Team to enhance systems and interfaces that provide real-time updates on vacancies for smoother placement.

Case Management Oversight
• Maintain oversight of rehabilitation and reintegration plans for residents in shelters and institutional care.
• Review cases to ensure residents are appropriately placed and assess requests for extensions of stay.
• Manage and update the central repository of data on residential care needs.

Secretariat and Administrative Support
• Provide secretariat support for the Review Committee and other relevant platforms.
• Plan, schedule, and coordinate meetings with various stakeholders.

Requirements

• Bachelor's Degree in any discipline.
• Prior experience in casework or case management preferred.
• Interest in working with vulnerable populations, including the poor, homeless, and destitute.
• A good team player with strong planning, interpersonal, writing, and computer skills.
• Candidates with an inclination to pursue or upgrade in social work will have an added advantage.
