Responsibilities:

• \tConduct pre-admission screening for patients and caregivers.

• \tSupport nurses in providing holistic care, including monitoring patient condition and progress.

• \tIdentify and report care gaps, issues, or abnormal findings to the clinical team.

• \tCommunicate regularly with patients and caregivers, providing updates and handling enquiries.

• \tParticipate in interdisciplinary team meetings and contribute to care planning.

• \tMaintain accurate documentation of patient care and progress.

• \tCoordinate with community partners and healthcare providers for continuity of care and referrals.

• \tAssist in developing care plans based on patient needs.

• \tMonitor patients' vital signs and escalate red flags to nurses/physicians.

• \tProvide basic patient education, lifestyle coaching, and health advice (e.g. diet, exercise, nutrition).

• \tEnsure adherence to telehealth monitoring protocols.

• \tEnsure availability of equipment and supplies, coordinating with vendors as required.

• \tParticipate in quality improvement, risk management, and departmental projects.

• \tCarry out other ad-hoc duties as assigned.

Requirements:

• \tDiploma in a health-related discipline preferred.

• \tPrior healthcare or hospital experience is an advantage.

• \tGood computer literacy.

• \tService-oriented, proactive, and adaptable in a fast-paced environment.
