• Minimum 3-5 years supervisory / management / leadership experience
• Experience in aged care / social service / therapy-integrated environment preferred
• Strong knowledge in policy development, quality management, and staff supervision
Brief Job Descriptions:
• Assist the Superintendent in the management of the Welfare Home in accordance with the Destitute Persons Act and Destitute Persons (Welfare Homes) Rules
• Plan, manage and control the safe and overall efficient delivery of quality healthcare services to the residents in compliance with the relevant legislative requirements as well as Standard Operating Procedures, Guidelines, Service Standards and directives issued by the authorities
• Responsible for carrying out preventive and curative medical services in the Home under the instructions of the medical doctor(s)
Key Responsibilities:
Management
• Responsible for the smooth and efficient operations of the Home's Nursing and Therapy Unit
• Ensure compliance with the requirements/guidelines/standards/Standard Operating Procedures on:
- Pharmacist's audit and audit responses
- Outbreak of disease
- Incident Reporting
- Medication management such as storage, packing, administration, disposal of medication and maintaining medical equipment
- Medical observation such as monitoring vital signs, blood glucose, observing conscious level for residents with altered mental status with regard to fall, measuring abdominal girth, bowel movement and urine output for residents with urinary catheter
- Administrative processes and procedures related to admission, discharge, hospitalisation, medical review/appointment and referral to medical institution
- Documentation requirements related to notification of infectious disease, drug allergy/adverse drug reaction status, change in treatment and management of medical condition reported in discharge summary, memorandums and/or resident assessment form
• Administer/Arrange/Coordinate/Monitor rehabilitation treatment, vaccination and health screening as may be advised by the authorities
• Put up recommendation for resident requiring nursing home placement
• Develop and implement Fall Management Plan, Chronic Disease Care Plan, Infection Control Plan, Fluid Restriction Plan and other health care management plans, where necessary
• Oversee the management of the residents placed in segregation/isolation rooms/dormitories
• Plan and coordinate the implementation of the Home's menu and Diet Management Plan
• Participate in and ensure compliance with the Home's Business Continuity Plan
Clinical Practices
• Audit pre-packed medication and Resident Medical Record daily
• Carry out daily rounds and supervise residents at mealtimes
• Manage emergency cases such as first-aid and treatment of fall
• Perform clinical role in the absence of staff nurse, arising from leave, training and/or vacancy.
• Supervise nursing staff on clinical practices
• Monitor and analyse performance indicators and suggest measures to improve practice/process
• Participate in clinical/service quality improvement activities
Administration
• Put up Incident Report, Case Review Report, Medical Report and any other report, as and when required
• Participate in the formulation and implementation of Individual Care Plan
• Maintain Medical Database
• Prepare duty roster for nursing team and re-allocate staff daily duty in case of shortage of manpower
• Responsible for the form control function of the Nursing and Therapy Unit
• Apply Medik Awas for residents, as and when required
Training
• Prepare and/or conduct orientation and in service training for new nurses and other staff
• Contribute to the preceptorship for newly employed nurses and the appraisal of nurses
• Recommend nursing staff for appropriate training programmes
• Prepare and/or conduct health education talks to residents
• Conduct competency assessment for Nursing Staff, as and when required
Finance and Logistics
• Maintain flu pandemic and haze stock at two months' supplies level or any other level as required by the authorities
• Monitor and purchase medical related items including supportive device, consumable and milk supplement
• Ensure appropriate approval and quotations are obtained prior to purchase of any medical equipment, items and consumables
• Prepare annual budget for medical related expenses
Other Duties
• Attend to member of Board of Visitors (Welfare Homes) or staff of Ministry of Social and Family Development during inspection/Operations audit
• Participate in committee meeting/sharing session/case conference and prepare and give presentation, as and when required
• Update the Home's doctor of any Medical Advisory by the Authorities, as and when required
• Perform any other duties, as and when assigned by the Superintendent
Work Location:
The Welfare Home provides care for destitute persons, and the work location will be at Buangkok Green.
• Thuja Home at Pelangi Village is a four-storey facility with a capacity of 239 beds, catering to female destitute persons. The workplace is located at Buangkok Green.
