- Location: Macpherson
- Salary: Up to $5000 - $6100
- Work Schedule: 5-days work week (Mon - Sun: 8.30 - 6pm |Fri: 8.30 - 5.30pm)
Key Responsibilities
- Provide leadership and supervision to a team of 3 to 6 social work professionals, ensuring high standards of case management, professional conduct, and collaborative team dynamics.
- Oversee end-to-end case management processes, ensuring that case files are submitted promptly, with accuracy and adherence to documentation protocols.
- Conduct regular case supervision sessions, offering professional guidance, clinical input, and mentorship to both staff and interns (e.g., art therapy, counselling, social work disciplines).
- Facilitate and chair monthly case conferences, and encourage ongoing case consultations and interdisciplinary discussions to strengthen care planning and intervention strategies.
- Serve as the key liaison with MSF and external partners to coordinate case interventions, escalate concerns, and activate appropriate services in a timely manner.
- Manage all aspects of ECRM, including reviewing and processing admissions, discharges, extensions, and case allocations within the team; ensure follow-ups are conducted consistently.
- Oversee the administration of the CANS tool, including staff account management, quality assurance of assessments, and resolution of technical or procedural issues.
- Review and vet critical reports such as RC4s and incident documentation; prepare and submit funding applications for residents assessed to be at higher risk or requiring additional support.
- Compile and analyze casework data for internal reporting, ensuring timely submission of monthly and quarterly statistics to support service reviews and audits.
- Maintain oversight of documentation and reporting systems (e.g., Muster spreadsheet), ensuring that all records are accurate, up-to-date, and aligned with compliance requirements.
Requirements
- At least 5 years of relevant experience.
- Diploma in Social Work, Psychology, Counselling, or related field.
