Responsibilities:

•             Responsible to handle patients in basic treatments

•             Educate patients on the disease progress

•             Encourage self-management

•             Identify at-risk patients

•             Perform some outcome management and tracking of data.

Requirements:

•             Diploma in Nursing from recognised institutions

•             Singapore Nursing Board recognised nurse / Registered with SNB
