Job Description
The Social Worker provides casework, interventions, and care coordination services to older persons and their caregivers within the home and community settings. The role supports ageing-in-place by addressing the psychosocial, emotional, and environmental needs of clients, facilitating access to care, and working in collaboration with CEF's multidisciplinary home care team and external partners. This role also supports community development efforts, leads group interventions, and participates in research and service improvement initiatives.

Role & Responsibilities
1. Psychosocial Assessment and Casework
- Conduct intake and comprehensive biopsychosocial assessments to evaluate clients' social, emotional, and financial needs.
- Develop, implement, and monitor individualised case and care plans to address psychosocial risks, caregiver stress, and safety concerns.
- Provide counselling, emotional support, bereavement care, and facilitate appropriate individual and/or family interventions.
- Perform home visits, means testing, and financial counselling as required.
- Assist clients in accessing financial aid schemes and community services through timely referrals.
- Maintain accurate and prompt documentation of interventions, progress, and reports.
- Ensure proper case closure upon achievement of sustainable outcomes.
2. Care Coordination and Service Integration
- Collaborate with allied health professionals, nurses, and care aides to ensure holistic care planning.
- Liaise with external stakeholders (e.g., AIC hospitals, MSF and community partners) to mobilise social and financial support.
- Assist clients in navigating applications for government schemes such as SMF, FDWG, Comcare, and Caregiver Support programmes.
3. Programme Development & Group Work
- Design and implement group work intervention plans to meet client needs.
- Supervise facilitation of group sessions and evaluate effectiveness.
- Identify service gaps and lead development of new programmes.
- Oversee programme implementation and outcomes evaluation.
4. Community Development & Outreach
- Conduct environmental scans and community needs assessments.
- Mobilise community resources and initiate development projects.
- Build and maintain strong relationships with community partners and stakeholders.
- Organise and lead crisis management efforts with external agencies.
- Support CEF's community outreach efforts to identify at-risk elderly in the community.
- Organise or participate in public education, caregiver training, and wellness initiatives.
5. Other Duties
- Assist the management team with special projects and ad-hoc assignments.

- Internal Contacts: Medical Social Worker, Social Work Associate, Nursing Team, Doctors, Administrative Staff
- External Contacts: Clients, Caregivers, Community Partners, Government Agencies
Qualifications and Skills:
Education & Professional Certification
- Degree in Social Work or equivalent recognised by the Social Work Accreditation and Advisory Board (SWAAB).
- At least 3-5 years of experience in medical social work within an acute or community care setting.
- Prior exposure to eldercare or services for vulnerable adults and community care landscape is an advantage.
- Experience working with clients with dementia, frailty, or palliative care needs is an advantage.

Working Hours:
- 5-day work week (Monday to Friday)
- 8:30 AM to 5:30 PM
- Off days: Saturdays, Sunday and Public Holidays (However, attendance at organization events on weekends may be required when neccessary.)
