Responsibilities:

• Identify, resolve, and prevent drug-related problems; recommend and monitor patient-specific therapies
• Participate in inpatient rounds and ambulatory care clinics to ensure safe, effective medication use
• Dispense medications and provide counselling to patients on their treatment plans
• Communicate accurate drug information and educate patients, public, and healthcare professionals
• Contribute to clinical or translational research to advance knowledge in specialty area

Requirements:

• Registered with the Singapore Pharmacy Council credited college of Pharmacy
• Registered with the Singapore Pharmacy Council
• Ability to work independently
• Able to work under fast paced environment
• Good listening and communication skills
