The Social Work Associate (SWA) supports the delivery of social work services by assisting with casework, group work, community outreach, and administrative functions. Working closely with internal teams and external agencies, the SWA contributes to improving clients' well-being through responsive, compassionate and professional support.

Working Hours:
The Social Work Associate (SWA) supports the delivery of social work services by assisting with casework, group work, community outreach, and administrative functions. Working closely with internal teams and external agencies, the SWA contributes to improving clients' well-being through responsive, compassionate and professional support.
Role & Responsibilities
1. Casework & Social Support Services
- Respond to enquiries via phone and email and provide appropriate information and referral services.
- Support intake processes, including monitoring means testing, conducting financial assessments, and carrying out home visits where required.
- Assist the Medical Social Worker (MSW) in preparing social reports and applying for financial aid schemes to support clients.
- Maintain accurate and up-to-date case notes and documentation for each client.
- Support in the development and implementation of social programmes and initiatives.
- Build rapport with community partners and stakeholders to facilitate coordinated care.
- Assist in data collection for research projects.
2. Senior Mobility Funding (SMF)
- Manage the end-to-end SMF application and approval process.
- Ensure accurate filing of SMF application documents in patient records.
- Create and update consumables accounts for clients receiving SMF devices.
3. Customer Service & Feedback Management
- Address feedback and complaints from clients and caregivers with professionalism and empathy.
- Maintain open and compassionate communication with clients, caregivers, and internal staff to foster trust and service satisfaction.
4. Administrative Duties
- Maintain and monitor referral and admissions data for reporting and statistical purposes.
- Provide general administrative support to the social work team and organization as required.
5. Other Duties
- Assist the management team with ad-hoc assignments and special projects as required.
Key Relationships
- Reports to: Medical Social Worker
- Internal Contacts: Medical Social Worker, Nursing Team, Doctors, Administrative Staff
- External Contacts: Clients, Caregivers, Community Partners, Government Agencies
Qualifications and Skills:
- Diploma in Social Work, Community Service, or relevant field; GCE 'A' / 'O' Levels with relevant experience will also be considered.
- 2-3 years of relevant experience in healthcare or community service settings preferred.
- Strong interpersonal and communication skills (written and verbal).
- Empathetic, compassionate, and able to work effectively with elderly clients and caregivers.
- Team-oriented with the ability to work independently when necessary.
- Well-organised, detail-oriented, and proactive.
- Proficient in Microsoft Office (Word, Excel, Outlook); comfortable using digital tools for scheduling and reporting.

Working Hours:
- 5-day work week (Monday to Friday)
- Working hours: 8:30 AM - 5:30 PM
- Off days: Saturdays, Sundays, and Public Holidays (However, attendance at organization events on weekends may be required when necessary.)
