Job Responsibilities:
· Deliver nursing care at patients’ homes, including procedures such as nasogastric tube replacement, urinary catheter changes, wound dressing, stoma care, medication compliance monitoring and packing, and blood glucose/parameter monitoring.
· Observe and report patients' conditions and progress to senior nursing staff in a timely manner.
· Educate patients and their families about the patient’s medical condition and promote understanding of holistic care as needed.
· Support the implementation of discharge plans based on assessed needs and assist with post-discharge care coordination.
· Conduct follow-up via phone reviews and home or agency visits to ensure necessary community services are arranged.
· Schedule home visits with clients and/or caregivers for new referrals.
· Perform and document nursing assessments appropriately.
· Develop individualized care plans based on assessment findings.
· Adhere to safety protocols and infection control measures during home visits.
· Collect payments and issue receipts to clients/caregivers after each visit, and submit collected payments to the Finance department accordingly.
· Carry out initial assessments to identify patient needs, including referrals for the Senior Mobility Fund (SMF), community services, or financial aid.
· Inspect and ensure the safety of all equipment used in home care.
· Handle administrative tasks as assigned by supervisors.
Job Requirements:

Diploma/ Degree in Nursing

Possess at least 2-3 years’ experience providing inpatient nursing care

Registered with Singapore Nursing Board

Possess Basic Cardiac Life Support (BCLS) + AED certification
