Job Description:

• Carry out home visits for chronic and palliative patients referred for care.
• Conduct nursing assessments during home visits.
• Collaborate with doctors for medical assessments and subsequent follow-up visits.
• Monitor patients' adherence to prescribed medications.
• Administer nursing procedures.
• Create and execute nursing care plans.

Requirements:

• Minimum Diploma / Degree in nursing
• Meet the immunisation requirements
• Valid SNB and BCLS
