Salary: Up to $4500 (Basic) + Bonus

Location: Eunos

Job Scope: 

• Conduct clinical assessments, care planning, and regular client reviews.
• Perform nursing procedures and ensure accurate documentation.
• Work closely with a multidisciplinary healthcare team.
• Provide psycho-social support and liaise with stakeholders on care needs.
• Manage daily homecare operations, enquiries, rostering, and emergencies.
• Supervise and train care staff, monitor performance, and handle staff issues.
• Lead quality improvement initiatives, audits, and incident management.
• Deputise the Care Manager in his/her absence.

Requirements:

• Min. Degree/Advanced Diploma/Diploma in Nursing (Bachelor’s degree in related field an advantage).
• Valid SNB and BCLS 
• Min. 3-4 years relevant experience, with proven department management and staff supervision.
• Strong clinical knowledge and performance management skills.
