Duration: Perm full time
Working Days & Hours: 5 days office hours
Location: Queenstown

Job Scope:

• Direct work with patients and families including intake interviews, conducting financial assessment and providing necessary assistance and assisting with referrals to facilitate discharge planning in selective areas of care planning
• Support Medical Social Workers in case management through translation assistance and is providing patient escort and home visit suppor
• Participate in work process improvements and quality improvement projects
• Work with other departments within the hospital and with external organizations to facilitate patient care and experience
• Provide support on the administrative and operational running of the department, including managing the counter and related duties
• Supporting the department in programmes and projects

Job Requirements:

• Min. Diploma in Social Work or any relevant field
