Responsibilities:
- Provide Advance Care Planning Facilitation to the public.
- Conduct assessment of client’s needs.
- Provide Bereavement Counselling and/or therapy to clients and families; prepares treatment plans, discharge plans, and follow-up care programs; provides crisis intervention and emergency services as required
- Monitor and evaluate the effectiveness of counselling interventions and adjust strategies as needed.
- Conduct Peace/Cloud Café sessions
- Develop and review appropriate protocols, polices and procedures, in care process, referral and assessment.
- Expand existing service counselling provision by promoting organization’s counselling and training services among other agencies and potential service users.
Requirements:
- At least a Bachelor’s Degree in Counselling.
- At least 3 years of clinical experience in counselling or social work, especially in elderlies and related sector.
- Passionate about providing early palliative care to elderlies.
- Excellent collaboration and teamwork related skills.
- This position is on contract basis.
