Counsellor (Community Palliative Care)

Responsibilities:

  • Provide Advance Care Planning Facilitation to the public.
  • Conduct assessment of client’s needs.
  • Provide Bereavement Counselling and/or therapy to clients and families; prepares treatment plans, discharge plans, and follow-up care programs; provides crisis intervention and emergency services as required
  • Monitor and evaluate the effectiveness of counselling interventions and adjust strategies as needed.
  • Conduct Peace/Cloud Café sessions
  • Develop and review appropriate protocols, polices and procedures, in care process, referral and assessment.
  • Expand existing service counselling provision by promoting organization’s counselling and training services among other agencies and potential service users.

Requirements:

  • At least a Bachelor’s Degree in Counselling.
  • At least 3 years of clinical experience in counselling or social work, especially in elderlies and related sector.
  • Passionate about providing early palliative care to elderlies.
  • Excellent collaboration and teamwork related skills.
  • This position is on contract basis.
