💰 Up to $2,700/month

Responsibilities:

• Coordinate and manage home care service requests

• Assess client needs and recommend suitable services

• Conduct home visits and follow-ups

• Liaise with internal teams and external partners

• Educate caregivers and support discharge planning

Requirements:

• Min. Diploma in Nursing/Social Work or related field 

• Good communication and interpersonal skills

• Passionate about community care
