Job Responsibilities:
- Process information and statistics using an electronic system and gather information for financial assessments.
- Conduct assessments and home visits when needed.
- Liaise with agencies, authorities, and patient families.
- Coordinate with nurses on patient-related matters.
- Supervise customer care coordinators handling subsidy cases.
- Act as a communication link between management and customer care coordinators.
- Maintain and update patient records.
- Stay updated on government policies regarding elderly care services and funding.
Job Requirements:
- Minimum O Level
- At least 1 year experience in healthcare industry
- Minimum GCE 'O' Level / NITEC / Diploma or equivalent.
- Prior experience in healthcare or customer service will be an advantage.
- Team player with a service-oriented mindset.
