Job Purpose

The Assistant Manager, Dementia Care Services is responsible for providing client care management and therapeutic treatment in collaboration with the dementia care team.

He/She will assist the Head of Department in overall operations and management of the facility, ensuring the delivery of high-quality person-centered care and services to the clients.

He/She performs care assessment, planning and interventions for the DCS clientele group in accordance with established YCC's policies, standards and evidence-based practices.

He/She takes up the role of Dementia Practitioner and provides expertise and knowledge in dementia care, develops and manages the programme of dementia care including the structured activities of dementia day care.

He/She supervises, educates and provides appropriate training to the dementia care team.

Key Responsibilities

Centre Management
  • Co-lead the team towards YCC's vision, values, objectives and achievement of targets set.
  • Manage daily operations of the Centre, ensuring compliance with licensing standards and monitor key performance indicators
  • Coordinate referrals to dementia daycare and monitor service utilization.
  • Collaborate with ComRels Department on donations, volunteers and including Integrated Community Care Partnership (ICCP)
  • Collaborate with key partners to plan, track and implement programs and schedules
  • Develop service standards, performance indicators and operating guidelines and implement operational strategies for the Services
  • Oversee centre logistics, procurement of supplies and manage relationships with suppliers, vendors and contractors
  • Handle all enquiries, feedback, complaints and incidents related to Centre operations
  • Maintain and update all Centre operations documentation for audit purposes.
  • Manage interactions with internal and external auditors and assist in updating Centre policies.
  • Oversee staff scheduling and ensure adequate coverage as needed
  • Plan, develop, implement and evaluate work procedures and / or processes in Dementia Care Services programmes and conduct periodical reviews to maintain / enhance standard of service provision.
  • Manage the implementation of programmes and services
  • Co-lead resolution of operational issues within the Services

Conduct client assessment and therapy planning
  • Conduct initial and periodical full functional assessments (set consisting various assessment forms stipulated) for all clients to determine required therapy interventions.
  • Develop individual care plan for all clients including SMART (Specific, Measurable, Attainable, Relevant, Timely) goals in collaboration with clients and relevant stakeholders.
  • Implement and review individual care plan for all clients.
  • Document results of case history taking, assessments, goals and outcome measures.
  • Conduct multi-disciplinary case conferences/discussions with members of the care team including relevant stakeholders including communicating assessment results, care goals and outcomes on a monthly basis/ as need arises.

Perform therapeutic interventions
  • Deliver appropriate interventions to facilitate client's needs based on findings, goals and care plans.
  • Review and modify care plans as appropriate.
  • Evaluate clients' participation in programme activities and manage accordingly.
  • Ensure comfort and safety in all clients.
  • Manage and escalate appropriately in the event of unexpected and/or abnormal changes.
  • Coordinate with relevant care staff or stakeholders to support clients in achieving their care goals.
  • Curate and review structured programme activities to facilitate clients' engagement in cognitive stimulation.
  • Manage behaviors of concerns with appropriate therapeutic interventions/ strategies.

Clients and caregivers empowerment
  • Communicate care plans, progressions and build rapport with clients and their families.
  • Involve clients, families, caregivers and/or relevant others as active partners, to identify needs, preferences and expectations of the clients.
  • Provide effective training, deliver dementia talks and education to clients, families and/or caregivers according to self-management needs.
  • Develop client and caregiver education materials.
  • Guide and support clients and caregivers in managing psychodynamics issues.

Programme management and operational excellence
  • Assess the eligibility of a potential client for admission to DCS programmes.
  • Maintain proper documentation of clients' progress and accurate collation of data (record of referral/ enquiry/ attendance etc).
  • Coordinate/Prepare statistics and reports monthly.
  • Direct, guide and participate in the orientation and training of new staff and provide ongoing training and monitoring of care staff in provision of care to clients with dementia.
  • Recommend initiatives and implements of quality improvement, evidence-based practice.
  • Maintenance of work environment and manage any issues as necessary.
  • Support early identification of health threat risks to clients through information and/or data gathered.
  • Comply with guidelines and policies to ensure client confidentiality and personal data protection.

Provide supervision and team development
  • Mentor support care staff for professional development purposes.
  • Plan induction and programme orientation for new team members.
  • Supervise and guide support care staff in care management of basic and complex cases.
  • Review care competency standards of support care staff and identify appropriate training needs.

Build stakeholder relationships
  • Collaborate with other organizations for the development of programmes
  • Manage communications and feedback from stakeholders
  • Develop community outreach plans to target client group, other organizations and the community
  • Liaise/ collaborate with ComRel department in management and development of volunteers, facilitation and coordination of VIA from school/ corporate groups, development and maintenance of corporate partnerships.
  • Establish positive and constructive relationship with other organizations delivering similar or related services.
  • Liaison with relevant statutory boards and/or government agencies on all service requirements of Bukit Merah and YEAH!'s programmes and provide reasonable feedback on the practicality of these requirements

Demonstrate corporate governance
  • Assist Senior Manager to implement governance policies and procedures for the Services
  • Assist Senior Manager to conduct audits on service quality to ensure compliance with policies

Manage risk and quality
  • Facilitates the implementation of emergency management plans
  • Assess risks of work systems, work processes and service delivery
  • Oversee implementation of preventive measures pertaining to risk management
  • Assist in formulating risk, emergency response and crisis management plans where necessary
  • Identify and report risks and barriers to the safety of clients in accordance with organisational protocols.
  • Execute responsibilities as per emergency protocols in the event of public health threat or emergency.

Manage health and safety standards
  • Conduct annual fire drill tabletop and fire drill exercise
  • Manage outbreak of Infectious Diseases
  • Review recommended preventive protocols and corrective measures
  • Monitor health, safety and medical emergencies in the centre.
  • Lead investigations of adverse events such as abuse or neglect of clients
  • Manage any internal or external incidents in DCS programmes according to SOP or as appropriate

Requirements for the position:
  • Candidates with a background in healthcare related field and a degree in nursing, social work, psychology, gerontology related areas
  • Minimum 3 years of relevant experience in healthcare setting providing care to Persons Living with Dementia.

  • Strong team player, resourceful and systematic
  • Knowledgeable and experienced in supervision and management
  • Effective written and spoken communication skills.
  • Effective interpersonal relationship building and employee coaching and development skills.
  • Ability to serve as a knowledgeable resource to the organization's management team that provides leadership and direction.
