Overview
An assistant case coordinator of Department of Orthopaedic Surgery assists care management of patients and clinical outcome team to coordinate and provide care activities and maintain the cluster’s clinical database for outcome tracking.


Job Responsibilities
- To assist care management team to coordinate and provide care activities for Orthopaedic Surgery patients.
- Perform basic preventive health screening and health assessment
- To assist clinical outcome team to coordinate and conduct patient’s survey and maintain the assigned database
- To closely monitor participants’ outcome, track their follow-up visits and ensure data collected within acceptable period
- To ensure health assessment for each patient is accurate and document into database is consistent with the source data
- To submit and present monthly report
- To collaborate with multidisciplinary team to improve clinical outcomes and patients’ satisfaction
- To attend lectures, courses and seminars where applicable
- To take on additional responsibilities as assigned by Reporting Officer.       


Requirements
- Diploma in Health Sciences, or State-Registered Nurse in medical/surgical nursing.
- Experience in patient care would be preferred.
- Able to demonstrate clinical skills, knowledge as well as the ability to handle multiple tasks.
- Possess strong interpersonal and communication skills.
- Proficient in MS office applications.
