Salary: up to $3000
Location: Central
Working hours: 5 days office hours

Responsibilities:

• Direct work with patients and families, providing necessary assistance 
• Support in case management through providing patient escort and home visit suppor
• Participate in work process improvements and quality improvement projects
• Facilitate patient care and experience
• Provide support on the administrative and operational running of the department
• Supporting in programmes and projects

Requirements:

• Min diploma in social work or a related field
• Prior relevant experience will be advantageous
