Job Description
•    To Implement patient care in collaboration with the multi-disciplinary teams and according to assessed needs and health condition.
•    To Facilitate admissions, transfers and discharges; ensures discharge planning is done timely upon patient’s admission and appropriate to condition.
•    To Follow up and evaluates patients’ progress according to planned care and readiness for discharge.
•    To Monitor patient for changes in condition, recognises early signs of clinical deterioration using appropriate physiological assessments and escalates as required to enable timely implementation of interventions.

Requirements
•    Advanced Dip / Degree in nursing 
•    At least 3 years of experience as Senior Staff Nurse in a clinical setting.
