URGENT* Club Facilitator (Contract) - Various locations

Working Hours: Centre operates Mondays to Fridays from 7am to 7pm
(Staff will be on staggered shifts, e.g 7am – 4pm, 8am – 5pm, 9am – 6pm, 10am – 7pm)

Job Scope:

• Provide personal care assistance to dementia clients in their daily living
• Perform tasks according to the care plans developed by clinical team
• Maintain up-to-date documentation of client’s conditions and interventions and support development and review of care plans
• Ensure client’s integration and recommend measure to mitigate risk to the clients
• Conduct activities for clients to promote independence, health, wellness and quality of life
• Provide guidance to clients on personal, social or work skills

Requirements

• Min GCE N/O levels
• Nitec in Community Care & Social services would be suitable!
• Training provided, must have passion to serve the elderly and comfortable to handle dementia patients
